Using CultureADD: Request Access

In order to use the CultureADD online assessment tool, you must formally request access. You will be asked to provide some basic organizational information, such as your:

  •Organization Type
  •Organization Name
  •Address, City, State, & Zip Code
  •Phone Number
  •Website
  •Date of Incorporation
  •Fiscal Year
  •Budget


As our main point of contact with your organization, you will be required to provide some information about yourself in the second part of the form. Once you submit the form, a CultureADD Administrator will review your information and contact you shortly. If your organization meets the criteria necessary to use the online assessment tool, access will be granted. If your request to use the tool is approved, you may proceed to payment processing, access the system and complete the questionnaire. Credit cards and debit cards with the Visa or Mastercard logo are the only forms or payment accepted. If you are not granted access to use the system, you will receive notification as to why that is the case.
